You may fax your prescription to CPAPScience at 713-589-4972 or you may upload your prescription using the form below.

Please reference your order number on your prescription, which you will receive on order confirmation email after purchasing a product.

Unless you have ordered an auto-adjust CPAP, be sure that the prescription indicates the pressure level of the device.
